Here is what to expect when you contact our office…
Melanie or Kelsey will complete a phone consultation to help us better understand the situation. We will discuss the current living situation, existing health conditions, any barriers to care and conduct a financial review. We will answer all of your questions and address any concerns, then we will discuss the tools needed to make a change.
The Team at Helping Hands believes that a personalized care plan that is unique to each family leads to the most long term success. After the initial consultation, we will compile a small customized list of services and community resources. We will then facilitate the connection to the appropriate resources.
To ensure that seniors and their families don’t fall through the cracks after their initial consultation, the Helping Hands Care Management Team follows up with families through monthly phone calls, email check-ins, or additional in-person consultations. We will leave no family behind because we feel that no one should have to take this journey alone.
